APPLICATION PROCESS REQUIREMENTS — Step by Step
1.Complete and submit Peers: Link to HOPE Initiative Application
To fill out an application you can either print off the application here Peers Link to HOPE Application or fill out the online form below (continue to scroll down to access the online application on this page). Hard copies of application can be sent via US Postal Mail to: Peers Link to Hope Program, Attn: Judy Benham, 3130 SE 2nd Ave, Grand Rapids MN 55744. 2. Submit 2 letters of recommendation via email to firstname.lastname@example.org, mail to the above address, OR fax to 218-999-4041. An application is not complete until both letters have been received. Letters of reference must be from a non-family member who can write about your mental health journey and/or vocational skill sets. A reference letter does not need to be longer than 1 page and must include the name of the applicant and contact information for the reference.
3. Once your full application and letters of reference have been received, you will be contacted by our PS Link Team within 2-3 weeks to set a up an phone interview.
4. Complete a phone interview with a member of the PS link Team (This can take up to 2-3 weeks from the time you submit you application and 2 letters of reference).
5. You will be notified by phone if you have been accepted into a training event. If you cannot be reached by phone, we will contact you via the email address you provided on your application.
Please note, we process 50-70 applications a month and training spots ARE LIMITED. Acceptance into a training event is not guaranteed until you have received a confirmation from our team about your acceptance status. The entire application process could take up to 4-5 weeks from start to finish.
